Please be sure to do the following:
- Contact your insurance company to see if you need a referral and/or authorization for your visit.
- Have your primary care doctor fax over the necessary referrals and/or authorizations before the day of your appointment to fax # 781-639-9017.
- Please bring with you copies of all your medical insurance cards.
- Please bring a valid picture ID with you.
- Any and all co-payments are due at the time of your appointment. We are unable to bill patients for co-payments.
On your first visit:
- In addition to the information above, please arrive 15 minutes prior to your first appointment to complete the required new patient information paperwork.
